Eileen Koons is director of Huntington Memorial Hospital’s Senior Care Network in Pasadena, California. SCN’s community-based programs include home and community-based care coordination, health education, information and resources for adults and older adults with disabilities and family caregivers.
She serves the aging and disability community through leadership and advisory roles locally and statewide including the MSSP Site
Association board as immediate past president, Azusa Pacific University social work department advisory board, USC Alzheimer’s Disease Research Center community advisory board, and Convalescent Aid Society board. She was awarded a Practice Change Fellowship 2009-11, for leaders in geriatric care funded by the John A. Hartford Foundation and The Atlantic Philanthropies. Previous roles include Los Angeles County Area Agency on Aging planning supervisor and planner; and she began her career as a hospital social worker. She graduated from the CSU Long Beach Master of Social Work degree program in the older adults and families concentration, and holds ACSW certification through the National Association of Social Workers.
